Abstract
OBJECTIVE: To assess the efficacy of perampanel in patients with GBM with refractory epilepsy. BACKGROUND: Malignant gliomas are known to release glutamate which is involved in tumor expansion and epileptogenesis. By blocking α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptors stimulated by glutamate, seizure control may be achieved. Perampanel, a noncompetitive AMPA glutamate receptor antagonist, has been shown to be an effective adjunctive treatment in patients with epilepsy. DESIGN/METHODS: We describe 3 patients with GBM and refractory epilepsy that achieved seizure control with the addition of perampanel to their current regimen. RESULTS: Case1. Patient is a 61 year old woman with recurrent seizures on levetiracetam and oxcarbazepine. Perampanel was added and the patient has been seizure free for over 3 months. Case 2. Patient is a 64 year old man with GBM and partial status epilepticus on valproic acid, levetiracetam and lacosamide. Perampanel was added and the focal seizures decreased significantly. The patient passed away after 5 weeks due to complications of GBM. Case 3. Patient 68 year old man with GBM status post 2 surgeries and epilepsia partialis continua on levetiracetam, valproic acid and carbamazepine. Perampanel was added and he has remained seizure free for over 5 weeks. CONCLUSIONS: Perampanel is efficacious and well tolerated as an adjunctive anti-epileptic drug in our patients with GBM and refractory seizures. Disclosure: Dr. Espinosa has nothing to disclose.
